Family Home Health is seeking a Home Health Clinical Manager to lead our clinical team in the greater Chicago area. The manager will oversee care coordination, supervise staff, and ensure high-quality patient care, safety, and satisfaction while maintaining documentation in our EMR system.

The ideal candidate holds a valid Illinois RN license with 5–10 years of home health nursing and at least 5 years in management, demonstrates outstanding communication, and can mentor staff and engage with

Qualifications

• Valid RN license in Illinois.
• Minimum 5-10 years strong home health nursing.
• Minimum 5 years of management and overseeing staff.
• Excellent observation, verbal and written communication skills.
• HCHB EMR experience a strong plus.

Responsibilities

• Manage the assigned home health clinical care team and assist with care coordination.
• Provide guidance on the interdisciplinary plans of care to ensure care and support needs are met.
• Train, mentor, and evaluate home health clinical care team on processes, protocols, compliance, and documentation requirements.
• Monitor clinical records to ensure visit, quality and satisfaction standards are met.
• Serve as liaison between field staff, referral sources, and home health operations.
• Evaluate and address patient, caregiver, and customer concerns expeditiously.
• Monitor visits that require supervisory oversight including home health aide and licensed practical nurse visits.
• Analyze required supervisory reports and document utilizing Agency’s EMR system, through a company-provided tablet.
• Attend all required interdisciplinary and staff meetings.
