Job summary

The Carter Burden Network is seeking an HDM Coordinator at the Carter Burden Luncheon Club to oversee home-delivered meals operations for older adults. You will manage intake, eligibility assessments, route planning, volunteer scheduling, and maintain client records in line with NYC Aging and CMOW standards.

Qualifications

• Ability to coordinate multiple tasks with accuracy and attention to detail.
• Experience coordinating meal delivery or social services is preferred.
• Strong written and verbal communication in English.
• Ability to work collaboratively with volunteers, drivers, and staff.
• Familiarity with NYC Aging standards and CMOW is a plus.
• Flexibility to work occasional evenings or weekends.

Responsibilities

• Coordinate all aspects of the Home Delivered Meals program, including intake, eligibility assessment, and reassessments.
• Ensure completion and accuracy of nutrition assessments conducted by staff or volunteers.
• Prepare and manage daily delivery route sheets and volunteer schedules, making real-time adjustments as needed.
• Maintain accurate client records and program data per NYC Aging and CMOW guidelines.
• Support audits by maintaining logs for taste tests, incidents, complaints, and surveys.
• Lead delivery crews in training and safety meetings.
• Prepare for emergencies and weather-related interruptions.
