Job summary

Qualifications

• Bachelor’s degree or equivalent experience.
• Minimum of 1 years’ experience working with the frail elderly population preferred.
• Current, valid NC driver's license and vehicle.
• Must be able to work effectively in a team environment.
• Excellent communication, interpersonal and conflict resolution skills.

Responsibilities

• Assess home care needs and develop specific plans of care with periodic updates.
• Coordinate 24-hour care delivery and implement home care services.
• Reconcile invoices for personal care service hours and supplies.
• Coordinate and authorize all Durable Medical Equipment (DME) and home supplies.
• Assist with discharge planning from nursing facilities and arrange appropriate care.
• Collaborate with Interdisciplinary Team and participate in staff meetings.
• Communicate with IDT regarding enrollees and care plans, including SMART goals.
• Work with clinic RN and PCP on in-home nursing needs; perform acute in-home visits as needed.
• Provide community resources with Social Worker support to meet participant needs.
