Job summary

Embark Care is seeking a Case Manager to supervise field staff and ensure high-quality client care across all services. You will oversee the full client care cycle including assessment, planning, implementation, monitoring, hiring, scheduling, and evaluation with a focus on satisfaction, retention, and cost effectiveness.

Qualifications

• Graduate of accredited High School.
• Minimum two years’ experience in health or home care positions.
• Must have strong communication, leadership, and ability to motivate people.

Responsibilities

• Plans, organizes, administers, and coordinates agency Client/Personnel functions.
• Serves as liaison among professional personnel and field staff.
• Reviews employee and client information to ensure qualified staffing."
• Evaluates environmental/working conditions of field employees during home visits and conveys concerns to Management.
• Scheduling, supervision and orientation of caregivers as needed.
• Maintain community standing and attend public events as needed.
• Oversee Start of Cares and Client Discharges.
• Document routine communication with employees and clients to ensure service quality.
• Ensure timely reporting of incidents, infections and client/caregiver concerns to Administration.
