Innovative Integrated Health, Inc. is seeking a Community Health Advisor to deliver health education to PACE-eligible seniors and their families within our service area.

You will support recruitment and enrollment for the PACE program, engage with community organizations, and represent IIH at outreach events and booths. The role requires strong communication skills, reliable scheduling, and the ability to manage outreach data.

Qualifikationen

• MS Office (Word, Excel, Access, PowerPoint, Outlook)
• Strong written and oral communication
• Ability to maintain confidentiality per HIPAA
• Excellent organization and time management
• Ability to build relationships with community organizations

Aufgaben

• Supports IIH's Business Development recruitment and enrollment for the PACE program.
• Participates in community health events and PACE-sponsored events.
• Develops relationships with community organizations (centers, churches, doctors’ offices, housing).
• Conducts outreach within the PACE service area to source marketing opportunities.
• Tracks outreach activity and submits weekly/monthly reports.
• Attends staff meetings, trainings, and committees as assigned.
• Maintains HIPAA confidentiality and professional boundaries.
• Flexible schedule; may require use of personal vehicle.
