Innovative Integrated Health (IIH) is seeking a Community Health Advisor in California to deliver health education to PACE-eligible seniors and families within the PACE model of care. This role focuses on outreach, enrollment growth, and forming strategic community partnerships.

The position requires strong MS Office skills, effective communication, and HIPAA confidentiality. Travel within the service area and flexible hours are common in this role.

Qualifications

• MS Office proficiency including Word, Excel, PowerPoint, and Outlook.
• Effective written and verbal communication.
• Ability to maintain confidentiality following HIPAA guidelines.

Responsibilities

• Supports IIH's Business Development recruitment and enrollment for the PACE program.
• Contributes to department goals, including growing and maintaining patient census.
• Participates in community health events and PACE-sponsored activities.
• Develops strategic relationships with community organizations.
