• Bachelor’s degree in public health, public administration, social work, health policy, or related field.
• 2+ years in supporting or managing community health or nonprofit initiatives.
• 3–5 years working with CBOs or healthcare providers, managing timelines and deliverables.
• Strong project management, timelines, milestones, and reporting capability.

Responsabilités

• Manage partnerships with community-based organizations, providers, and stakeholders for Connect2Care and health equity initiatives.
• Develop and monitor project plans, timelines, milestones, and deliverables; track risks and dependencies.
• Coordinate meetings, trainings, and stakeholder events to strengthen partnerships.
• Monitor performance with data collection, KPIs, dashboards, and regular leadership reporting.
• Support program data collection, communications, and program expansion activities.
