Job summary

Mass General Brigham Population Health seeks a GRACE Social Worker for in-home geriatric assessment and care management aligned with primary care providers. The role focuses on proactive, patient-centered management for high-risk older adults in a hybrid Boston-area setting.

The position is full-time (40 hours) with 36 hours of clinical care and 4 hours of admin time, combining home visits in the Boston/Boston Metro West area with remote work for the rest of the week.

Qualifications

• MSW degree required from an accredited program.
• Driver's license required.
• 2+ years of clinical experience in a medical setting.
• 1+ years of geriatric experience preferred.
• Clinical licensure (LCSW/LICSW/LMHC) preferred.

Responsibilities

• Screening and ongoing psychosocial assessments.
• Develop and implement comprehensive care plans.
• Coordinate with GRACE ID Team and primary care providers.
• Provide education on community resources and advance directives.
• Advocate for clients to access needed care and resources.
• Collaborate with team and other service providers.
• Provide supportive counseling and crisis intervention as needed.
• Maintain accurate documentation and performance improvements.
