Job summary

Scharp is seeking a Lead Case Manager for the FSP-ICM program in Los Angeles. This role provides frontline operational support, field guidance to case managers, and coordination of housing, transportation, and community resources to ensure continuity of care for clients.

The Lead Case Manager will supervise and train staff, ensure adherence to program guidelines, and collaborate with interagency teams to deliver recovery-oriented, whole-person rehabilitation services in community settings.

Qualifications

• Bachelor’s degree in Social Services or related field with 2 years of adult mental health experience
• Medi-Cal Peer Support Specialist training, or willingness to complete within 6 months
• Valid California driver’s license and access to a car

Responsibilities

• Lead weekly staff meetings to review cases and documentation
• Model outreach and engagement activities per guidelines
• Assist field staff with difficult-to-engage clients and training
• Coordinate housing linkage, transportation, and resource navigation
• Ensure continuity of care across hospitals, shelters, and community settings
• Provide field-based mental health rehab and targeted case management
• Deliver onboarding and ongoing training for staff
• Maintain updated community resource guide
• Collaborate with shelters, housing providers, and agencies
• Support crisis intervention and 24/7 on-call coverage
