Ultipro is seeking a Lead Case Manager to guide field staff delivering FSP-ICM services, ensuring outreach, engagement, and housing linkage for clients. You will supervise, train, and support case managers and peer staff while maintaining rigorous documentation and billing standards.

The role emphasizes collaboration with shelters, hospitals, and community providers, requiring strong leadership, communication, and crisis-response skills to improve client outcomes in a community-based setting.

Qualifications

• Bachelor’s degree in Social Services or related field with 2 years’ relevant experience; or Associate degree with 3 years; or HS diploma/GED with 3–4 years.
• Certificate of Medi-Cal Peer Support Specialist training or willingness to complete within 6 months.
• At least two years’ experience in full service partnership or intensive case management.
• One year of management/supervisory experience preferred but not required.
• Pass California DOJ background check, Live Scan and/or fingerprinting.
• Valid California driver’s license and access to a car with full coverage insurance.
• Ability to conduct outreach and engagement for difficult-to-engage clients.
• Knowledge of community resources, housing systems, rehabilitation services.
• Ability to collaborate with interdisciplinary teams, shelters, hospitals, and providers.
• Strong communication, organization, and field coordination skills.
• Ability to provide leadership and guidance to other staff.
• Ability to respond quickly to client/family crises and life events.

Responsabilités

• Conduct weekly meetings with staff to review cases and documentation, guiding client care and policy adherence.
• Assist with outreach and engagement activities per guidelines, modeling activities as needed.
• In the field, support staff with difficult-to-engage clients and challenging situations.
• Coordinate housing linkage, transportation, resources, and other linkage needs.
• Ensure continuity of care for clients transitioning from hospitals, shelters, or institutional settings.
• Participate in weekly client screenings and field-based eligibility reviews for FSP-ICM.
• Deliver field-based mental health rehabilitation and targeted case management alongside staff needing training.
• Conduct onboarding training and ongoing education for case management and peer staff.
• Maintain and enhance community resource guides and linkage information.
• Coordinate with shelters, housing providers, and community agencies for client support.
• Ensure staff implement recovery-oriented, strengths-based rehabilitation services.
• Meet billing targets and documentation standards for assigned cases and outreach work.
