Job summary

The People Concern's Senior Case Manager (FSP) role supports wraparound, field-based services for individuals with complex needs, including serious mental illness and homelessness. You will lead housing efforts and connect clients with supportive services in collaboration with the FSP team.

Based in the Wellness Center in Los Angeles, the position is on-site, Monday–Friday, 8am–4:30pm, with participation in meetings, trainings, and on-call duties as needed.

Qualifications

• Bachelor’s degree in related field, or an equivalent combination of education and experience.
• Three (3) years or more of Case Management experience.
• At least one (1) year of experience working with adults with severe, persistent mental illness and co-ocurring substance use disorders, and with chronically homeless individuals.
• Current, valid CA Driver’s license, reliable personal vehicle with auto insurance, and willingness to transport clients in personal vehicle.
• Experience and familiarity with LA County resources, benefits, and services to help clients meet their case management needs.
• Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills.
• Strong interpersonal and team building skills.
• Knowledge of Public Housing Authority and Housing subsidy preferred (Section 8, Shelter Plus Care, VASH).

Responsibilities

• Provide linkage to stable housing, and other supportive services as needed to assist clients to obtain housing stability, in addition to client advocacy, case management, and benefit establishment.
• Identify appropriate permanent housing options for clients and support them through the process such as by conducting screening interviews, completing housing documentation, and coordinating clients’ move-in with the housing authority and property managers.
• Perform initial comprehensive case management assessment and develop a plan of intervention that addresses their barriers, increases their income, and assists them in maintaining permanent housing.
• Encourage and promote an environment that is strength based to assist clients in meeting their individual goals.
• Complete all documentation and maintain productivity expectations for billable time, according to standards and as they are required by our funder (the Department of Mental Health).
• Actively participate in staff meetings and trainings.
• Assist in crisis interventions as needed, including participating in on call rotation calendar for overnight and weekend After Hours Emergency Line.
