Job summary

CVS Health is seeking a Senior Epic Analyst for Healthy Planet and Care Coordination. The role leads design, build, and optimization of Epic solutions to support population health management, care workflows, and coordinated care delivery.

You will mentor junior analysts, partner with care management and clinical leadership, and translate program strategy into scalable Epic solutions across transitions of care and cross-continuum communication.

Qualifications

• Bachelor’s degree in Health Informatics, Information Systems, Nursing, or related field (or equivalent).
• 4+ years of Epic experience, with substantial hands-on work in Healthy Planet and/or care coordination/care management modules.
• Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory or related modules preferred.
• Experience designing workflows for population health, care management, or value-based care programs.
• Strong understanding of transitions-of-care processes and cross-continuum care delivery.

Responsibilities

• Design, build, and maintain Healthy Planet registries, care plans, care teams, and health maintenance topics.
• Configure and optimize risk stratification tools, care gap identification, and outreach workflows.
• Lead testing and validation of Healthy Planet builds.
