Job summary

The Center for Family Support Inc. is seeking a Sr. DSP Medical Coordinator who will perform all Direct Support Professional duties plus coordinating medical appointments, labs, testing and documentation.

The role emphasizes accurate medical tracking, incident reporting, and adherence to regulatory requirements. Ideal candidates have HHA/DSP experience, CNA background preferred, and a willingness to maintain meticulous records and coordinate with nurses and managers in a supportive community

Qualifications

• HS Diploma or GED is required.
• 1+ year of HHA experience required; CNA experience a plus.
• 2+ years of Direct Support Professional experience a plus.

Responsibilities

• Document all paperwork required, including incident reports, log entries and Res. Hab. progress notes when appropriate.
• Track all medical appointments and follow-up appointments and document in tracking forms.
• Track all lab tests and ensure correct documentation in medical tracking log.
• Document all consults, refusals and MD recommendations appropriately.
• Ensure medication inventories are completed at least weekly and order new medications as prescribed.
• Maintain medication storage area in accordance with regulations and communicate errors to nurse/manager.
