Job summary

Providence at Home with Compassus seeks a Clinical Care Partner III to coordinate transitions of care for hospitalized patients and evaluate home-based care needs. This in-person role emphasizes bedside engagement, interdisciplinary collaboration, and accurate documentation within EMR systems.

Qualifications

• Active and unencumbered RN, LCSW, or Speech Therapist license for state of employment.
• Experience with discharge planning, care transitions, and post-acute coordination preferred.
• Experience working with EMR systems (Epic) and referral platforms.

Responsibilities

• Coordinate safe, efficient hospital-to-home transitions and discharge planning.
• Evaluate post-acute home-based care needs and arrange services.
• Educate patients and families on post-acute options and care implications.
• Collaborate with physicians, case management, and post-acute providers.
