Job summary

Compassus Providence at Home with Compassus is seeking a Clinical Care Partner to coordinate safe, efficient, patient-centered transitions of care for hospitalized patients. You will evaluate patients for appropriate post-acute home-based care services and support timely discharge planning in collaboration with physicians, case management, patients, families, and post-acute providers.

Qualifications

• Education Required: Associate’s degree in Nursing, Health Sciences, or related field.
• Preferred: Bachelor’s degree in nursing, Health Sciences, or related field.
• Experience: None required; 2–3 years preferred in care coordination, discharge planning, or healthcare services.

Responsibilities

• Coordinate safe post-acute transitions and discharge planning with hospital staff.
• Evaluate referrals and determine appropriateness for home-based care services.
• Educate patients and families about post-acute options and care pathways.
• Develop and implement transition-of-care plans with clinical goals.
• Coordinate hospice or GIP referrals when applicable and ensure proper documentation.
