Job summary

Qualifications

• Bachelor's Degree in Social Work or in Human Services is required.
• MSW degree is preferred for advancement.

Responsibilities

• Establish course of action by exploring options and setting goals with resident and family.
• Assist residents in achieving the highest practicable level of self-care, independence and well-being.
• Provide medically related social services to attain or maintain wellbeing.
• Evaluate social and family information to develop care plans for social services.
• Provide emotional support and address problems for residents and families.
• Assist residents with health care decisions and discharge planning.
• Coordinate with community agencies to meet resident needs.
• Document resident social service status and arrange on-site services as needed.
• Counsel residents and family members.
