Qualifications

Responsibilities

• Oversees Care Coordination staff and services to ensure high-quality, safe, and cost-effective patient care.
• Directs recruitment, onboarding, performance evaluations, and staff scheduling within the department.
• Leads budgeting, goals, and objectives for Care Coordination aligned with organizational targets.
• Promotes interdisciplinary collaboration and quality improvements through data-driven initiatives.
