Qualifications

• Must be ADA Registered Dietitian.
• Must have a valid Driver's License.
• Must have one year's experience working specifically with an elderly or aging population.

Responsibilities

• Develop comprehensive nutritional plans for each member, in line with their care plan.
• Implement nutrition-based care plans and supplements, then evaluate outcomes.
• Visit in-home members to conduct intakes and dietary planning, then ongoing visits/evaluation.
• Communicate nutritional plans to physicians, nurses, family members and caregivers.
• Ensure best practices in LIFE Center kitchen and food prep areas, including sanitation.
• Manage and work within the nutritional budget.
• Collaborate with the interdisciplinary team and advocate for members' nutrition needs.
