Job summary

The Salvation Army of the Syracuse Area seeks a Social Day Case Manager to support the Social Day/Day Hub and Congregate Meal Site, handling intake and ongoing assessments, case management, and coordination to ensure high-quality, person-centered services for seniors.

This full-time role offers meaningful work in a mission-driven team with comprehensive benefits and on-site parking. A background in Human Services and a NYS driver’s license are preferred.

Qualifications

• Associates Degree preferred.
• Three to five years of related experience in Human Services.
• Valid NYS Driver’s License.

Responsibilities

• Conduct participant intake, evaluations, and ongoing assessments.
• Manage an assigned caseload of seniors and maintain client files.
• Provide referrals, advocacy, service coordination, and support.
• Communicate needs and plan changes to program staff and provide training.
• Assist with calendars, schedules, and programs for clients.
• Maintain attendance, billing, and other required documentation.
