Job summary

The Community Health Program Manager II at L.A. Care Health Plan leads the design, implementation, and continuous improvement of programs that advance health equity for priority populations in Los Angeles.

You will conduct research, analyze data, build partnerships, and oversee program planning, monitoring, and budget management. This role requires strong communication, stakeholder engagement, and the ability to translate strategy into scalable operations across the Community Health department,

Qualifications

• At least 5 years’ work experience in health care or a related field, doing problem solving, policy, and/or program implementation.
• Experience building relationships with organizations and business partners.
• At least 1 year of experience in leading program teams.
• Demonstrated written and verbal communication skills; ability to synthesize data.

Responsibilities

• Lead design, implementation, and improvement of health equity programs and initiatives.
• Oversee program goals, budgets, and performance metrics; manage scope and timelines.
• Engage internal and external partners to align goals and resolve issues.
• Develop KPIs and evaluation plans; monitor performance with dashboards and reports.
• Ensure regulatory compliance and document audits and reporting requirements.
