Job summary

LA Care Health Plan is seeking a Community Health Program Manager II in Los Angeles to lead design, implementation, and continuous improvement of health equity initiatives for priority populations. You will conduct research, analyze data, build partnerships, and lead program planning for improved managed care performance and member outcomes.

The role oversees program goals, KPIs, budgets, and stakeholder alignment, ensuring quality standards and cost-effective care.

Qualifications

• Minimum 5 years’ experience in health care or related field with program delivery.
• Experience building relationships with organizations and business partners.
• Experience leading program teams (1+ year).

Responsibilities

• Formulate and implement solutions to moderately complex program challenges.
• Monitor operations for quality, efficiency, and effectiveness; manage budgets.
• Lead stakeholder engagement and cross‑functional collaboration.
• Build and maintain relationships with internal and external partners.
• Develop program goals, KPIs, and evaluation plans with dashboards.
• Support external initiatives and ensure regulatory alignment.
• Deliver educational presentations to varied audiences.
• Perform other duties as assigned.
