University Hospitals in Ohio is seeking an advanced Community Health Worker to work largely independently with Care Transitions staff, UH patients, providers and partner sites.

You will educate patients, link them to community resources, coordinate referrals, and support care coordination with social workers and the care team.

Qualifications

• High School GED or equivalent required.
• Bachelor's degree preferred in Social Work, Public Health or related field.
• 1+ years as a Community Health Worker in a clinical or community setting (Required).
• Experience with multiple healthcare data sources (EMR, registration, scheduling, claims) (Required).
• Experience facilitating telephonic patient outreach (Required).

Responsibilities

• Provide patient education and connect patients with resources within Care Transitions, UH, and community.
• Coordinate with patients to assess community resources and referrals.
• Interact with Social Worker, RN, and care team to facilitate patient care coordination.
• Maintain PHI/HIPAA compliance and protect patient confidentiality.
• Maintain daily/weekly tasks, documentation, and follow-up communications.

Education

High School diploma or GED

Bachelor's degree in Social Work, Public Health or related field
