Job summary

University Hospitals in Ohio seeks an advanced Community Health Worker to work largely independently with Care Transitions, UH patients, providers and partner sites to educate patients and connect them with resources.

You will coordinate with patients to assess needs, facilitate referrals, address barriers to attendance, protect PHI, and collaborate with social workers and care teams to promote coordinated care.

Qualifications

• Strong verbal and written communication skills.
• Ability to protect PHI and maintain confidentiality.
• Experience with healthcare data sources and EMR access.
• Proficiency with Microsoft Office.

Responsibilities

• Educate patients and connect to resources within Care Transitions and UH.
• Coordinate with patient to assess needs and referrals for community resources.
• Collaborate with social workers, nurses, and care teams for care coordination.
• Address barriers to attendance and ensure PHI security.
