• BSN preferred.
• Minimum 3 years in Home Health, Discharge Planning, or Case Management.

Responsabilidades

• Coordinate clinical care for a designated patient population across the continuum of care.
• Engage in discharge planning and transition of care to ensure timely, appropriate settings.
• Collaborate with medical, nursing and ancillary staff to optimize quality and cost-efficient outcomes.
• Manage clinical resource usage and LOS in collaboration with the care team.
• Provide ongoing consultation and education to staff on discharge and home care processes.
