Job summary

OhioHealth is seeking a Community Health Worker to assist patients facing non-clinical barriers to care. You will work with a multidisciplinary team to identify obstacles, coordinate support services, and connect patients with community resources to improve health outcomes.

You will conduct intake interviews, document interactions, and help patients adhere to care plans while upholding HIPAA standards and engaging with local organizations to extend care beyond the clinic.

Qualifications

• High school diploma or equivalent.
• Community Health Worker Certification by an approved training program or started within 1 year of hire.
• Experience working with under privileged populations in the community.

Responsibilities

• Direct patient support (60%): Meet individuals, conduct intake, identify non-clinical barriers, may include home visits, coordinate support services and community resources, promote health education, and support treatment adherence.
• Communication with health care team (20%): Document interactions, maintain relationships with community resources, participate in care team huddles, report patient demeanor and relevant contextual information.
• Administrative tasks (20%): Maintain patient records, serve as community liaison, handle phone triage, referrals, and ensure appropriate action.
