Join Optum Home & Community Care's Senior Community Care (SCC) team as a Per Diem Nurse Practitioner serving high-risk Medicare members across home, assisted living, and long-term care settings. This integrated care role focuses on comprehensive assessments, advanced illness management, care coordination, and interdisciplinary collaboration to improve outcomes for medically complex senior populations. Position Highlights

• Per diem role supporting high-risk members in the Senior Community Care (SCC) program.


• Coverage need is primarily afternoons and weekends.


• Expected commitment is approximately 24 hours per month.


• Provide care in home, nursing home, assisted living, and senior housing environments.


• Conduct comprehensive assessments, medication reviews, and advanced care planning discussions.


• Collaborate with an interdisciplinary care team to support complex patient needs and care transitions.

Required Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.


• Active, unrestricted Nurse Practitioner license (or ability to obtain before start date).


• Active DEA licensure/prescriptive authority (or ability to obtain post-hire) and ability to establish a collaborative practice agreement if required.


• Active driver's license and reliable transportation.

Compensation for this specialty generally ranges from $52.64 - $78.85 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
