Qualifications

• Active and unrestricted license in the state of residence.
• Certified Nurse Practitioner or Physician Assistant with board certification.
• Current DEA license or ability to obtain post-hire.
• Driver's license and access to reliable transportation.

Responsibilities

• Collaborate with primary care physicians to provide care.
• Work with nursing staff and patients' families.
• Collaborate with case management team.
• Documentation, coding and gap closure.
