Job summary

Optum Senior Community Care is seeking an NP or PA for per diem work in the Lewis and Thurston Counties, WA area. The role involves performing a one-time comprehensive health assessment in skilled nursing facilities and supporting care coordination to improve patient outcomes and reduce readmissions.

The ideal candidate will be licensed, certified (AANP/ANCC or NCCPA), able to travel to SNFs within the territory, and available to commit at least 10 hours per week with a flexible schedule

Qualifications

• Must hold an active, unrestricted NP or PA license.
• Certification via AANP/ANCC (NP) or NCCPA (PA).
• Active DEA license or ability to obtain.
• Valid driver's license and reliable transportation.
• Ability to travel to SNF facilities within the territory.
• Per diem, minimum 10 hours per week.
• Collaborative practice agreement may be required by state law.

Responsibilities

• Conduct comprehensive health assessments for patients in skilled nursing facilities
• Collaborate with primary care physicians, nursing staff, families, and case management teams
• Support care coordination and transition planning
• Complete documentation, coding, and gap closure activities
• Help improve quality measures, advanced care planning, and patient satisfaction
