Job summary

UnitedHealthcare is seeking a Clinical Transformation Consultant to lead physician practice transformation across ACOs and population health programs. You will drive improvements in quality, utilization, and cost while coordinating with senior leaders and practice teams. Remote work options available within IL/WI.

The role requires strong data analytics, reporting, and stakeholder management to achieve contract milestones and enable impactful care delivery.

Qualifications

4+ combined experience with improving clinical quality, health care analytics, or driving clinical transformation initiatives with population health programs, PCMH or ACOs.
Experience interpreting clinical data analytics and outcomes to drive change.
Experience communicating with senior leadership and presenting findings.

Responsibilities

Accountable for deploying clinical support program at the practice level, including reporting tools and dashboards.
Build and maintain relationships with practice leadership and clinical influencers.
Lead transformation meetings to monitor progress on action plans and milestones.
Develop strategies based on performance data to improve quality, efficiency and utilization.
Educate and deploy technology tools to support practice transformation and integrate into workflows.
