UnitedHealthcare is seeking a Senior Clinical Transformation Consultant to lead clinical transformation across ACOs, PCMH, and population health programs. You will guide practices, mentor colleagues, and drive quality, efficiency and cost improvements while reporting progress to leadership.

Remote work flexibility is available for eligible candidates in the St. Louis and Springfield, MO area, with travel as needed to client sites to achieve milestones and sustain transformation across networks.

Qualifikationen

• Bachelor's degree or 9+ years of experience in clinical quality, analytics, or transformation.
• 5+ years in clinical quality improvement or population health transformation.
• 2+ years interpreting clinical data analytics and driving change.

• Deploy clinical support programs at the practice level and educate on tools and reporting.
• Build relationships with practice leadership and drive transformation milestones.
• Lead and facilitate practice improvement meetings and monitor progress.
• Develop data-driven improvement strategies and monitor outcomes.
• Integrate technology tools into practice workflows and collaborate with teams.
• Travel 10-15% for face-to-face meetings as needed.
