Job summary

Qualifications

• Pharm.D. degree and Louisiana license.
• 4–5 years in inpatient hospital or clinical pharmacy setting.
• Board Certification in applicable specialty strongly preferred.

Responsibilities

• Directs and maintains clinical pharmacy services.
• Provides clinical consultation to the Medical Staff and other health care providers.
• Documents all activities/interventions in medical records.
• Develops treatment guidelines and formulary changes as approved by committees.
• Participates in pharmacy and therapeutics committees and assures regulatory compliance.
