Job summary

Cleveland Clinic's Main Campus is seeking a Clinical Pharmacist - Transplant, PRN, to join the Inpatient Pharmacy-Main Campus Hospital. The role supports the Lung Transplant Program with decentralized clinical pharmacy services, collaborating with transplant physicians, nurses and the multidisciplinary team to optimize immunosuppressive therapy and drug monitoring.

The position involves working various shifts in a PRN capacity, with a focus on evidence-based care, patient safety, and education

Qualifications

• PharmD or Bachelor's of Science in Pharmacy from an accredited college of Pharmacy.
• Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
• Current Basic Life Support (BLS) Certification through the AHA or Red Cross and completion of an immunization course within 90 days of hire for immunizers.

Responsibilities

• Provide decentralized clinical pharmacy services for inpatients.
• Partner with transplant physicians, APPs, nurses and the care team to optimize therapy.
• Focus on immunosuppressive therapy management and therapeutic drug monitoring.
• Engage in direct patient care and provide evidence-based medication recommendations.
• Educate and train students, residents and other caregivers.
