Job summary

LCMC Health is seeking a General Clinical Pharmacist to work with physicians, care managers, and clinical staff on patient outcomes while maintaining cost. You will provide therapeutic assessments and disease management for assigned practices within a medical care team.

The role involves direct patient care activities, collaboration with health care providers, development of guidelines, and active participation on clinical committees to improve care delivery and safety.

Qualifications

• Required: Pharm.D. Completion of PGY 1 Residency Program.
• Preferred: PGY 2 in Critical Care or Emergency Medicine OR Board Certification in Critical Care (BCCCP) or Emergency Medicine (BCEMP).

Responsibilities

• Directs development and maintenance of clinical pharmacy services.
• Provides clinical consultation to the Medical Staff and other health care providers within the appropriate team or unit(s).
• Communicates and documents all activities/interventions (medication orders, dosing, ADRs, errors).
• Fosters teamwork and ensures delivery of high‑quality patient care.
• Participates on clinical committees and liaises with physicians, nursing, and other healthcare professionals.
• Develops and implements treatment guidelines, pathways, and formulary changes.

Education

Pharm.D. + PGY1 residency

PGY2 in Critical Care or Emergency Medicine or Board Certification (BCCCP/BCEMP)
