Job summary

CAI is seeking a Senior Clinical Lead to provide vision and leadership for the National Technical Assistance Program, guiding health centers in improving chronic disease management, nutrition, preventive care and operational effectiveness.

The role leads clinical strategy, develops TA materials, and collaborates with evaluation, communications and partner teams to deliver impactful guidance. Hybrid work is offered with NYC preference.

Qualifications

• Registered Nurse (RN), Nurse Practitioner (NP), Physician Assistant (PA), or Physician (MD/DO) with a relevant clinical degree.
• 10+ years of leadership of improvement initiatives in primary care or health centers.
• Experience applying evidence-based interventions and team-based care models in primary care.
• Knowledge of improvement frameworks and implementation science.

Responsibilities

• Serve as CAI's senior clinical leader and national SME on chronic disease management and preventive care.
• Lead design and delivery of technical assistance to improve access, quality and outcomes in primary care.
• Define and advance models for chronic disease management, population health and team-based care.
• Lead efforts to strengthen care continuity, follow-up and partnerships for patient supports.
• Develop TA materials, tools and playbooks; coordinate with content and evaluation teams.
• Present findings to funders and internal stakeholders and support dissemination.
