Job summary

CAN Health in Boston is seeking a Clinical Lead for Chronic Care to define care pathways and partner with hospitals and payers on CAN Platform initiatives.

You will review AI outputs for safety, contribute to HIPAA/FDA regulatory work, and ensure products reflect real-world clinical practice. A licensure-backed clinician will lead cross-functional efforts with a focus on value-based care.

Qualifications

• Clinical degree and active licensure (MD, DO, NP, PA or equivalent).
• 3+ years of clinical practice with exposure to health tech or digital health.
• Experience in chronic disease management and care pathways.
• Familiarity with value-based care, remote monitoring, and care coordination workflows.
• Excellent written communication and ability to translate clinical details for non-clinical teams.

Responsibilities

• Define and maintain clinical protocols for chronic condition pathways (cardiovascular, diabetes, respiratory).
• Partner with hospital and payer customers to co-design care programmes on the CAN platform.
• Review AI-generated content for clinical accuracy, scope, and safety.
• Contribute to regulatory and compliance workstreams (HIPAA, FDA Digital Health, payer credentialing).
• Serve as a clinical voice in product planning to ground features in real care delivery.
