Job summary

WelbeHealth PACE is hiring an LVN/LPN to provide nursing care in our clinic for frail, homebound seniors. You’ll perform wound care, administer medications, monitor vitals, and coordinate with PCPs and the care team in a value-based model.

Requires an unencumbered LVN/LPN license and at least 1 year of elderly patient care, plus CPR/First Aid within 180 days of hire. We offer comprehensive benefits and a supportive, patient-centered environment.

Qualifications

• Graduate of an Accredited School of Nursing with an unencumbered LVN/LPN license.
• Minimum 1 year nursing experience in a clinical setting with frail or elderly population.
• Nursing knowledge to care for frail, elderly participants and manage complex clinical situations.
• Highly motivated, self-directed and able to adapt in a fast-changing environment and make sound decisions in emergencies.
• Excellent clinical, organizational and communication skills with seniors, families and interdisciplinary teams.
• Highly organized and able to juggle multiple patients.
• CPR and First Aid certification within 180 days of hire; certification on hire preferred.

Responsibilities

• Perform and document wound care in clinic as directed by PCP.
• Perform and document procedures and care, including medication management and administration.
• Administer medications to participants in center and reinforce education.
• Coordinate with provider on medication order changes from same-day procedures; oversee changes as directed.
• Perform medication review during Welbe Welcome Day for new visits.
• Perform pre- and post-operative clinical touchpoints and follow-ups; escalate as needed.
• Maintain flexibility as business needs evolve.
