Job summary

WelbeHealth La Quinta, CA is seeking an LVN/LPN to provide direct nursing care to participants in our clinic. The role includes wound care, medication administration, vital signs, and coordinating with PCPs.

Qualifications

• Graduate of an Accredited School of Nursing with an unencumbered LVN/LPN License required
• Minimum of one year nursing experience with frail/elderly population preferred
• Strong clinical, organizational, and communication skills in elder care settings
• CPR and First Aid certification within 180 days of hire preferred

Responsibilities

• Perform and document wound care in clinic in accordance with scope of practice
• Administer medications and reinforce medication education
• Coordinate with providers on changes and oversee medication orders
• Perform medication review/reconciliation during participant visits
• Conduct pre/post operative touchpoints and follow-ups as directed by RNs/PCP
