Job summary

WelbeHealth is seeking a Home Health LVN to deliver skilled nursing care in participants’ homes in the Los Angeles area under CMS oversight. You will work under RNs, NP or MDs, perform home visits, manage medications, and coordinate with family and clinicians to ensure safe care transitions.

Requirements include graduation from an accredited nursing program, an active LVN license in California, reliable transportation, CPR/First Aid within 180 days, and flexibility to a varied schedule including

Qualifications

• Graduate of an Accredited School of Nursing required.
• Unencumbered LVN License in state practicing.
• Reliable means of transportation.
• Nursing knowledge and skills necessary to treat frail, elderly participants and manage complex clinical situations.
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Responsibilities

• Provide and document in-person supervision of home health aides and assistants in participants' homes.
• Instruct participants, families and caregivers on disease processes, self-care techniques and prevention.
• Conduct and perform home safety assessments as assigned.
• Deliver and document nursing interventions, respond to physician orders, administer medications and perform tests and treatments.
• Support care transitions between settings (hospitals, SNFs, home).
• Be available for varied schedules including evenings, nights, weekends and overtime.

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. The Home Health LVN plays a vital role in achieving our mission by performing skilled duties at home under CMS approval, bringing convenient and necessary care to our participants’ homes.

Under the supervision of WelbeHealth RNs, NP or MDs, the Home Health LVN is responsible for carrying out tasks in the participants’ residences as assigned such as home safety assessments, data gathering and supervisory visits. The Home Health LVN will perform skilled duties only after site licensure or in the event of pandemic/stay at home order with CMS approval to provide necessary care in the home.

Essential Job Duties:

• Provide and document in-person supervision and instruction to home care assistants and home health aides assigned to participants’ homes at least as frequently as specified in the Home Health Agency regulations and more often if necessary
• Instruct the participant, family and caregivers regarding the disease process, self-care techniques, and prevention strategies, as assigned
• Conduct and Perform home safety assessments as assigned
• Deliver and document nursing interventions as assigned, accurately responding to physician orders, and correctly administering medications and performing ordered tests and treatments
• Support to the clinic staff and home health staff work to manage smooth care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
• Must be willing and could work a varied schedule that may include evening nights, weekends and overtime

Job Requirements:

Compensation consists of base salary plus bonus. WelbeHealth offers a competitive total rewards package that includes a 401(k) match, comprehensive healthcare coverage, and a broad range of additional benefits. Actual compensation will be determined based on experience and relevant qualifications.
