SCAN Group is hiring an LVN/LPN to deliver on-site and field-based care to Embrace members in assisted living and community settings under NP/MD direction. You’ll perform focused assessments, wound care, vaccinations, IV therapy, catheter care, and coordinate with the care team to maintain high-quality care.

Qualifications

• Licensed Vocational Nurse (LVN) license required or equivalent certification.
• Associate's Degree or higher preferred or equivalent experience.
• 3–5 years of related healthcare experience.
• Able to perform under direct supervision with escalation of complex issues.

Responsibilities

• Perform focused clinical assessments including vital signs, pain, and basic cardiopulmonary/GI observations.
• Provide wound care, vaccinations, IV therapy within scope, catheter care, and routine foot care.
• Administer vaccinations, injections, and other treatments per protocol.
• Conduct follow-up checks after transitions or acute events as directed by the clinical team.
• Communicate proactively with the primary care team and residents’ families; coordinate care.
