Job summary

Health Advocacy Team Support (HATS) is seeking a Community Health Worker Lead to guide a CHW team focused on Medi-Cal member services. The Lead coordinates workflows, coaches staff, reviews documentation, and ensures member needs are resolved, bridging CHW services with ECM, CS, Intake, and partners.

The role supervises CHWs, maintains a caseload of up to 10, and partners with the Program Manager and HR to support compliant, person-centered care while upholding confidentiality and quality

Qualifications

• CHW Certification required and annual training compliance.
• Lived experience with Medi-Cal target community per standards.
• Experience in CHW navigation or community health with coaching skills.
• Bachelor's or Master's preferred in public health or related field.
• Proficient with electronic health records and standard office software.
• English fluency required; bilingual English/Spanish preferred.

Responsibilities

• Guide daily work and staff performance; approve schedules and coach staff.
• Monitor caseloads and continuity of services; review intake assessments and goals.
• Support member engagement; provide culturally responsive, trauma-informed CHW services and advocacy.
• Coordinate eligibility and approvals; verify Medi-Cal eligibility and care plan reviews.
• Review documentation; complete service notes within 24–48 hours and audit for required details.
• Support compliant billing and audit readiness with billing staff and providers.
• Coordinate ECM and Community Supports referrals; verify enrollment dates to avoid duplicate billing.
• Train staff; onboarding, shadowing, and annual continuing education.
• Report results; track engagement and referral metrics and identify workflow barriers.
• Protect member information; escalate urgent concerns and privacy issues promptly.
