Pacific Health Group is seeking a CHW Program Manager to lead a team of CalAIM Community Health Workers delivering whole-person care to Medi-Cal members across San Diego County, CA. The Program Manager ensures high-quality service delivery, operational efficiency, and strict adherence to CalAIM billing and documentation standards.

The role requires strong leadership, training, and cross-functional collaboration with RNs and local partners to achieve optimal care coordination and outcomes.

Qualifications

• Bachelor’s degree in Public Health, Social Work, Healthcare Administration or related field.
• 3–5 years in community health or Medi-Cal services with supervisory experience.
• Strong knowledge of CalAIM policies and social determinants of health.
• Valid California Driver’s License with vehicle and auto insurance; CHW cert preferred.

Responsibilities

• Oversee, mentor, and evaluate a team of CalAIM CHWs delivering outreach, health navigation, group facilitation, and advocacy for high-risk populations.
• Ensure CalAIM billing compliance and CPT coding standards; enforce service unit limits.
• Audit member Plans of Care, progress updates, and standardized assessments across portals.
• Coordinate onboarding and ongoing education, including Motivational Interviewing and advocacy training.
• Collaborate with RNs and community organizations to streamline care coordination.
