Health Advocacy Team Support (HATS) is seeking a Care Team Manager in California to lead a high-performing Enhanced Care Management team serving individuals experiencing homelessness and complex medical needs.

You will guide care plans, amplify client voices, and ensure services reflect lived experiences. The role emphasizes equity, advocacy, and empowering teams to deliver person-centered care.

Qualifications

• Bachelor's degree required in a health/social field.
• 3-5 years of care coordination or case management, with leadership experience.
• Understanding of housing instability and equity.
• Experience navigating health systems and community services.

Responsibilities

• Supervise and mentor Enhanced Care Management team.
• Oversee development of person-centered care plans for individuals facing homelessness and chronic conditions.
• Build relationships with shelters and community partners.
• Track outcomes and advocate for flexible care models.
• Support caseloads when workforce is stretched.
• Address systemic barriers like mental health access and housing safety.
• Lead internal trainings on equity and homelessness.
• Conduct performance evaluations for team members.
