Job summary

Greenbrook Medical is expanding a patient-centered primary care model for seniors in the Tampa Bay area, offering high-touch care with a focus on prevention, chronic disease management, and personalized care plans. You’ll work within a team of care coordinators and medical assistants to deliver comprehensive care in a collaborative setting.

Physicians will manage a panel of around 500 patients, with 30-45 minute visits and a schedule designed for meaningful patient interactions.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related primary care specialty.
• Board certified or board eligible.
• Active Florida medical license or ability to obtain one.
• Experience in primary care, managed care, value-based care, or residency.
• Strong clinical judgment and excellent communication skills.

Responsibilities

• Build lasting relationships with patients and caregivers.
• Deliver comprehensive primary care focused on prevention, chronic disease management, and healthy aging.
• Create personalized care plans to improve quality of life and long-term health outcomes.
