Job summary

Brooksville, United States welcomes a Primary Care Physician to join a senior-focused practice emphasizing value-based care and long-term patient relationships.

The role offers open compensation with a base of $300,000 plus up to $120,000 in annual bonuses, signing bonuses, generous PTO, CME allowance, and a clear path to partnership after 2 years, with relocation support and robust clinical staffing.

Qualifications

• MD or DO degree from an accredited medical school.
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics.
• Experience in Managed Care, Value-Based Care, or Medicare Advantage preferred.
• New Graduates Welcome.
• ACN licensed accepted.
• Strong communication and patient engagement skills.
• Spanish-speaking ability preferred for one opening.

Responsibilities

• Provide comprehensive primary care services to Medicare Advantage and senior patients age 65+.
• Perform thorough assessments and develop individualized care plans.
• Manage chronic conditions, preventive care, and acute medical concerns.
• Deliver patient-centered care within a managed care/value-based care model.
• Collaborate with physicians, specialists, and interdisciplinary care teams to coordinate treatment plans.
• Identify and close care gaps while supporting quality and outcome initiatives.
• Educate patients and caregivers regarding disease prevention, healthy lifestyle choices, and long-term wellness.
• Build strong relationships with patients and families through compassionate, personalized care.
• Maintain accurate and timely documentation within the EMR.
