TEEMA Group is seeking an Outreach Assistant to support Senior Center enrollment and community engagement in San Francisco. You will help connect older adults with health education resources, assist with enrollment, and coordinate administrative tasks across workshops and programs.

Qualifications

• 1–2 years of outreach, enrollment, and program administration.
• 1–2 years of health promotion, health education, and outreach.
• Valid California Driver's License with good driving record.
• Bilingual English/Spanish or English/Chinese.

Responsibilities

• Participate in community outreach events throughout San Francisco to generate referrals for health promotion and healthier living programs.
• Attend community events, including health fairs, cultural festivals, faith-based gatherings, and other networking opportunities.
• Promote health education workshops through email campaigns, social media, and community partnerships.
• Engage with community members to support recruitment and enrollment in health education programs.
• Complete the enrollment process for patients participating in health promotion and healthier living programs.
• Recruit, enroll, and follow up with individuals participating in health education workshops.
• Complete all required data collection, documentation, and paperwork throughout the enrollment process.
• Maintain accurate and up-to-date databases documenting enrollment and outreach activities.
• Maintain enrollment-tracking spreadsheets and program records.
• Support the delivery of health education workshops focused on chronic disease self-management, diabetes, pain management, HIV, and related health topics.
• Receive cross-training to assist with workshop facilitation when needed.
• Host virtual classes and support workshop operations.
• Attend departmental meetings and assist with program deliverables and documentation required by funders.
