Qualifications

• 1–2 years of outreach, enrollment, and program administrative assistance.
• 1–2 years of health promotion, health education, and outreach.
• Bilingual English/Spanish or English/Chinese required.
• Valid California Driver's License.

Responsibilities

• Participate in community outreach events throughout San Francisco to generate referrals for health promotion and healthier living programs.
• Attend community events, including health fairs, cultural festivals, faith-based gatherings, and other networking opportunities.
• Promote health education workshops through email campaigns, social media, and community partnerships.
• Engage with community members to support recruitment and enrollment in health education programs.
• Complete the enrollment process for patients participating in health promotion and healthier living programs.
• Recruit, enroll, and follow up with individuals participating in health education workshops.
• Maintain enrollment-tracking spreadsheets and program records.
• Support the delivery of health education workshops focused on chronic disease self-management, diabetes, pain management, HIV, and related health topics.
