Job summary

Catholic Charities, Diocese of Trenton is seeking a dedicated case manager to deliver case management and counseling services to clients and families in the Rapid Re-housing program. This role focuses on personalized support and outcome-driven planning.

You will design, implement and monitor individual plans; assess eligibility; counsel clients; maintain records; coordinate activities and connect participants with housing, employment services, and related resources.

Responsibilities

• Designs, implements and monitors individual program plans for clients.
• Assesses client needs; investigates their eligibility for services.
• Counsels and aids individuals and families requiring agency assistance.
• Maintains client records and completes required documentation according to agency and accreditation standards.
• Coordinates client activities.
• Assists clients with accessing other services (job training, counseling, housing, medical services, etc.).
• Networks with other providers to generate community resources for participants.
• May provide supportive counseling in individual and group settings.
• May transport consumers to appointments, etc.
• Trains other case managers with regard to best practices for work products such as case tracking and data entry in Homeless Management Information System.
• Provides counsel to other case managers in duties associated to the provision of case management services.
• Acts as program liaison between case managers and Director.

• Coordinates client activities.
• Assists clients with accessing other services (job training, counseling, housing, medical services, etc.).
• Networks with other providers to generate community resources for participants.
• May provide supportive counseling in individual and group settings.
• May transport consumers to appointments, etc.
• Trains other case managers with regard to best practices for work products such as case tracking and data entry in Homeless Management Information System.
