Job summary

Women In Need, Inc. (Win) is seeking a Case Manager – Supportive Housing in New York to deliver trauma-informed, comprehensive case management for families in permanent supportive housing.

You will assess, plan, and monitor services to promote housing stability and long-term self-sufficiency. Qualifications

  • Conduct comprehensive family assessments within 48 hours of program admission and complete intake documentation.
  • Develop, implement, and monitor individualized service plans and ILPs to promote housing stability and long-term self-sufficiency.
  • Provide ongoing case management, crisis intervention, counseling, and trauma-informed care.
  • Advocate for clients and connect them to public benefits, healthcare, mental health services, education, employment programs, and community resources.
  • Assist families in understanding lease agreements and financial management to maintain housing and prevent rental arrears.
  • Monitor progress, conduct follow-up meetings, and document in CARES and other required systems.
  • Collaborate with property managers, interdisciplinary teams, and community partners to support housing retention and family well-being.

THE ROLE

The Case Manager – Supportive Housing provides comprehensive, trauma-informed case management services to families residing in permanent supportive housing. This position plays a critical role in helping families maintain housing stability, strengthen self-sufficiency, and achieve personal, educational, employment, and financial goals.

What You Will Do
  • Conduct comprehensive family assessments within 48 hours of program admission and complete all required intake documentation.
  • Develop, implement, and monitor individualized service plans and Independent Living Plans (ILPs) that promote housing stability and long-term self-sufficiency.
  • Provide ongoing case management, crisis intervention, counseling, and supportive services using trauma-informed and strengths-based practices.
  • Advocate for clients and connect them to public benefits, healthcare, mental health services, education, employment programs, and community resources.
  • Assist families in understanding lease agreements, tenant responsibilities, and financial management strategies to maintain housing and prevent rental arrears.
  • Monitor client progress, conduct regular follow-up meetings, and maintain accurate documentation in CARES, Awards, the Uniform Client Chart, and other required systems.
  • Collaborate with property managers, interdisciplinary teams, and community partners to support housing retention, recertification requirements, employment readiness, and overall family well-being.
Who You Will Be
  • Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred with two (2) years of relevant experience.
OR
  • Associate degree with three (3) years of direct client-facing human services experience.
  • Experience working with supportive housing, homeless services, family services, or related human services programs.
  • Knowledge of trauma-informed care, housing retention strategies, and case management best practices.
  • Experience working with diverse and vulnerable populations.
  • Familiarity with public benefits systems, housing programs, and community resources.
