Qualifications

• ASN with 5 years of RN case management experience in lieu of BSN accepted.
• 2+ years in case management or utilization management.

Responsibilities

• Coordinate services to maximize patient outcomes and ensure effective discharge planning.
• Assess, evaluate, and coordinate patient care with service lines and care teams.
• Serve as the liaison for patient, family, health care team, and insurance personnel.
• Document assessments and patient interactions in the EHR per department standards.
• Mentor New Graduate Nurses and supporting staff through continuing education and quality improvements.
