Qualifications

• Bachelor's Degree in Nursing or Associate's Degree in Nursing with 5 years RN case management experience
• California RN license is REQUIRED
• 2 years RN experience in case or utilization management REQUIRED
• ACM/CCM certifications preferred

Responsibilities

• Coordinate services to maximize patient outcomes and develop discharge plans
• Coordinate utilization management and discharge planning
• Serve as contact person and facilitator for patient, family, and care team
• Document assessments and patient interactions in EHR
• Support professional development and quality improvement initiatives
• Activate emergency response when needed
